The VNA of Cape Cod in conjunction with Cape Cod Healthcare Rehab Services are providing FREE fall risk assessment screenings throughout Cape Cod.

  • Truro Community Center: 11/6/23, 12:00 PM to 2:00 PM
  • Chatham Senior Center: 11/29/23, 10:00 AM to 12:00 PM
  • Harwich Community Center: 11/29/23, 1:00 PM to 3:00 PM
  • Bourne Senior Center: 12/13/23, 10:00 AM to 12:00 PM
  • Wellfleet Adult Community Center: 1/8/24, 10:00 AM to 12:00 PM
  • Brewster Senior Center: 1/8/24, 1:00 PM to 3:00 PM
  • Dennis Center for Active Living: 1/11/24, 1:00 PM to 4:00 PM
  • Sandwich Center for Active Living: 1/17/24, 10:00 AM to 12:00 PM
  • Provincetown Veterans Memorial Comm. Center, 10:30 AM to 12:30 PM
  • Eastham Senior Center: 1/29/24, 1:30 PM to 3:30 PM

Participants must be a resident of the respective town. Registration is required: no walk-ins will be accepted.
