Cape Cod Health Care’s Community Health Needs Assessment Provides Critical Information
Every two years, Cape Cod Hospital, Falmouth Hospital and their parent organization Cape Cod Healthcare (CCHC) prepare a Community Health Needs Assessment which provides critical information on significant health needs of Barnstable County residents. The purpose of the Assessment is to: identify health needs, engage stakeholders, and select significant health issues and identify implementation strategies. The synthesized health and demographic data are useful to any organization applying for grants, collaborating with other organizations to fill gaps in services, and seeking a deeper understanding of Cape Cod’s health and human service needs.
