Barnstable County’s ‘Prevention and Wellness Trust Fund’ In Action

In 2014, Massachusetts Governor Deval Patrick announced first-in-the-nation Prevention and Wellness Trust Fund (PWTF) which will provide more than $40 million in grants to nine community-based partnerships over four years to help fight chronic illness and improve health outcomes while reducing health care costs.

Forward two years, the Barnstable County Department of Human Services is managing the implementation phase for five local partners:  three clinical partners (Community Health Centers of Cape Cod, Duffy Health Center, Harbor Community Health Center – Hyannis) and two community partners (Healthy Living Cape Cod, YMCA of Cape Cod).  Barnstable County’s PWTF Partnership is in the implementation phase of evidence-based health interventions in the communities of Barnstable, Mashpee, Falmouth, and Bourne, and is focused on chronic disease prevention of diabetes and hypertension and prevention of falls among older adults.  To learn more about the state grant, visit the Massachusetts Executive Office of Health and Human Services PWTF.
