Navigator Pilot Project Grant Award
The Department of Human Services is pleased to announce that it has awarded a $90,000.00 grant to the Visiting Nurse Association of Cape Cod for an 18 month Navigator Pilot Project. The purpose of the project is to improve access to information and the coordination and delivery of home based long term services and supports. The project will assist residents of Cape Cod aged 18 and over who have chronic health conditions and disabilities, regardless of income.
The Navigator Pilot Project will assist individuals and their families navigate the long term services and supports system to access needed services. The Navigator is intended to enhance access to existing services and will be offered to individuals who require intensive short term advocacy and assistance to ‘navigate’ multiple service systems so they have the services and supports to remain as independent and healthy as possible. The Navigator Pilot does not duplicate existing programs and services.
The VNA has a long history of providing community based services and were chosen through a competitive RFP process. Over the next several months, the VNA will begin to conduct outreach to organizations and groups on how to access the Navigator Project. Funding for the grant is made possible by Barnstable County, CHNA 27, and Cape Cod Health Care.
