Barnstable County Department of Health and Environment is seeking a full-time Care Resource Coordinator/Reportable Disease Case Investigator. This grant-funded position involves conducting disease case investigations and supporting public health programs under the direction of the County Public Health Nurse and the department’s Director. The role requires collaboration with health teams and includes community health education and support group involvement. The position offers a hybrid work model with occasional travel and evening or weekend hours. Candidates must be able to handle confidential information with discretion, be proficient in Microsoft Office, and have an associate degree with at least two years of experience in a medical or social work field (a bachelor’s degree in a health-related field is preferred).
