5 Cape Organizations Coordinated by Human Services Receives Award
On June 13, 2017, the Barnstable Partnership of the statewide Prevention and Wellness Trust Fund (PWTF) initiative was awarded a Certificate of Excellence by the Massachusett’s Department of Public Health. The local partnership of five Cape Cod organizations is coordinated by Barnstable County’s Department of Human Services and is recognized for the following achievements:
- Greatest increase in A1C control of PWTF patients at risk for or suffering from diabetes
- Excellent fiscal stewardship and innovative project management
- Highest number of clients completing diabetes and pre-diabetes self-management programs.
