New Outreach Program for Homeless Youth
Homeless Youth Outreach is a new program offered by the Homeless Prevention Council (HPC) that aims to support youth ages 16 – 24 who are in need of stable and safe housing, community resources and a network of support. Through community outreach, case managers will coordinate supports with youth groups, associations, schools, and other organizations. The long-term goal of the program is to create connections to resources and community groups while providing comprehensive case management to provide solutions and address long-term needs. For youth under the age of 18, HPC will coordinate case management services in collaboration with the parent or guardian or will refer youth to an appropriate support service for minors.

The Homeless Prevention Council is a member of the Cape and Islands Regional Network on Homelessness Policy Board which is convened by the Barnstable County Department of Human Services. The Regional Network is a broad-based public-private partnership committed to identifying and implementing creative solutions to preventing and ending homelessness on Cape Cod, Martha’s Vineyard, and Nantucket.
