Cape Cod Healthcare FY15 Annual Strategic Grants

Cape Cod Healthcare (CCHC) released its FY15 Annual Strategic Grant Request for Proposals (RFP). This RFP is part of an annual competitive grant process focused on addressing unmet health needs of the residents of Barnstable County. CCHC will issue $250,000 in Strategic Grants for the upcoming year. The grants program is open to all community organizations with programs aligned with the identified Community Benefits priorities.

The FY15 Annual Strategic Grant RFP will concentrate on specified chronic and infectious diseases, and mental health – two of the six significant health needs identified by the 2014-2016 Cape Cod Hospital and Falmouth Hospital Community Health Needs Assessment (CHNA) Report and Implementation Plan.

To qualify for a FY15 Strategic Grant, proposals must meet at least one of the following two guidelines:

  • Address and positively impact specified chronic or infectious disease management among the following: Cancer, Cardiovascular disease, Diabetes, Hepatitis C, HIV/AIDS, and Tick-borne illnesses.
  • Support initiatives targeted at individuals and families facing mental health issues by addressing any of the following mental health concerns: High or increasing rates of depression, anxiety and suicide; Use of hospital emergency departments for psychiatric care; Declining availability of outpatient psychiatric care; and Access to and navigation of available mental health services in the region.

Grant proposals must address at least one of these specified chronic or infectious diseases or mental health issues, and meet funding guidelines related to vulnerable populations and measurable outcomes.

Proposals are due by September 10, 2014. Grant selection will be finalized in November 2014 by the CCHC Community Health Committee. The funding period runs from January 1 – September 30, 2015.
